Please bring the following documents with you when you enroll your child(ren):
 

• Social Security Card
• Birth Certificate
• Proof of Address (Rental agreement, Electric Bill, etc.)
• Shot Record
• CDIB (if applicable)
• Proof of Parent’s Citizenship (Birth Certificate, Valid Oklahoma Driver’s License, etc.)
